Skilled Nursing
St. Joseph’s Home Health, Inc. provides skilled nursing services to patients who need continuous medical care right in the privacy and safety of their own home. These services are administered by a registered nurse or a licensed practical (vocational) nurse under the supervision of a registered nurse. Before beginning our services, a qualified health advocate will go over the services we offer and which might best help you achieve your goals.
Physical Therapy
St. Joseph’s Home Health, Inc. provides physical therapy designed to enhance, restore and maintain the movement and physical abilities of a patient due to physical injuries and disabilities. Our physical therapists will work closely with you and your physician to formulate the most suitable therapeutic plan and programs balanced according to your unique health condition.
Occupational Therapy
Occupational therapy is used to help patients improve and restore their cognitive, physical and motor skills essential in performing everyday activities. Our licensed occupational therapists will provide customized therapeutic and exercise programs to restore your function in doing daily living activities.
Speech Therapy
At St. Joseph’s Home Health Inc., we will help patients overcome their language and speech difficulties. Our pathologist will assess the patient’s rehabilitation needs including the severity of his/her speech disorder. We will also provide training to the patient, family or caregivers to augment speech-language communication, treatment and to establish an effective maintenance program.
Medical Social Worker
Our medical social services are available to all patients who need social assistance and counseling. We are able to provide a qualified medical social worker or a social worker assistant to resolve social or emotional problems that are an impediment to the effective treatment of the patient’s medical condition or rate of recovery.
Home Health Aide
In addition to skilled nursing services, we also provide home health aide. St. Joseph’s Home Health, Inc. has a team of home health aides who provide services to patients right in their own home.
In order for a patient to qualify for home health aide, he/she must be confined to home and under the care of a physician. The services are under a plan of care that is established and periodically reviewed by a physician. Also, the services must be reasonable and necessary to the treatment of the patient’s illness or injury.
Provider Services
St. Joseph’s Home Health, Inc. offers quality non-skilled care which is intended to help family, friends, caregivers and their loved one who need assistance with the activities of daily living (ADL). We provide this service right in the comfort of your home based on the assessment and personalized plan of care.
Private Duty Care
Based on your assessment and plan of care, Private Care will help patients to perform every day activities. We will work with you or your loved one to assess their needs and formulate a unique plan for them.
Private duty care is available to anyone 18 years or older, whether you have a chronic illness or need a caregiver to supervise you in your recovery. Private duty covers a range of assistance services from personal care to more instrumental activities such as household organization. Our CNAs prevent fall risks, help patients with their rehabilitation, and oversee overall safety.
